His work is based on the use of micrograft and minigraft hair transplantation. A micrograft is a section of one or two hair follicles or bulbs; a minigraft is a section of three or four bulbs. The surgery yields a natural looking head of hair and no visible scars. Using local anesthesia, the surgeon removes an elliptical flap of scalp, containing between one thousand and one thousand five hundred minigrafts and micrografts, from the upper neck region of the head. Dr. Uebel notes that roots harvested from this area exhibit a superior life span to those found in other areas of the scalp. After suturing the donor site, the surgeon uses a micro-surgical knife to cut the section of excised scalp into strips of approximately one or two millimeters in thickness. Each of these strips is separated into micrografts and minigrafts. Then, using a jewelers forceps, the surgeon implants these grafts in punctiform incisions made in the bald area of the head. Additionally, an erbium laser can also be used to achieve more density without bleeding. "The entire procedure can be completed in two or three hours. There are no stitches involved in planting the grafts, and in most cases, patients can return to work the next day," explains Dr. Uebel. Following surgery, patients must wear a dressing for 48 hours, after which they can shampoo their heads, using an antiseptic shampoo. During the first two weeks, there is a crust formation which will fall off as it dries. One of the distinctive aspects of the surgery is that it transplants hair follicles rather than hair. As a result, patients will not see actual hair growth for two or three months. In the event a denser head of hair is desired, patients can repeat the procedure. "The patient can repeat the process at one-year intervals. This allows the follicles from previous grafts to fully germinate and reduces the risk of disturbing those transplanted before," notes Dr. Uebel. The micrograft and minigraft procedure can be used with all types of baldness, but it is especially appropriate for individuals who experience loss around the temples and incipient baldness. For patients with more extensive baldness, the surgeon and patient should also discuss the advantages of more traditional methods of transplantation such as using entire flaps of scalp en masse. While this method can help ensure a more noticeable result in a relatively short time, the use of micrograft and minigraft transplantation can be expected to achieve a better quality long-term result. One of the more common, associated with patients that have oily scalps, is the formation of small oil pustules and cysts that can be readily treated.
